In addition to on-site coaching, Summit Health also offers telephonic coaching by our professional staff of health educators and nurses. We provide telephonic coaching for the following:

On-site Health Screening Follow-up

When requested, Summit Health will follow-up with on-site screening participants to:

• Verify that those individuals who signed a Medical Referral Form called their primary care physicians to share their biometric screening results.
• Provide coaching on the results from HRAs and the on-site screening.
• Provide periodic support for those participants who have chosen self-directed lifestyle management efforts.
• Report results from home test kits and to provide coaching on appropriate next steps.
• Report results from off-site lab tests and to provide coaching on appropriate next steps.
