Coaching is included with all on-site health screenings. For those clients who wish to offer additional coaching, Summit Health provides telephonic services that include:
1. Medical referral follow up
2. Review health risk assessment (HRA) and health screening results
3. Periodic reinforcement of self-directed lifestyle management efforts
Summit Health’s on-site screening process identifies employees who should contact their Primary Care Physician (PCP). Employees whose test results fall outside of recommended guidelines are asked to sign a commitment that they will share their results with their PCP. Summit Health provides an optional service whereby Summit Health medical staff follows up with employees via telephone to answer questions they may have and ask whether they shared their results with their PCP. Employees who have not done so are encouraged to share their results with their PCP.
A number of clients integrate HRAs with their health screenings. In most cases HRAs are completed by employees after the health screening and employee do not have an opportunity to be coached on the results nor ask questions.
Summit Health provides an optional service whereby our medical staff returns to the site (usually two to three weeks after the health screening) to provide coaching to employees on both the HRA and health screening results. Coaches review steps taken, accomplishments, and barriers to achieving goals, revising wellness goals if appropriate. These coaching sessions usually take 15 to 30 minutes and can be scheduled by employees using the Summit Health proprietary online appointment system.
Attendance at follow up coaching can be tracked and reported electronically to one or more incentive programs.
Follow up telephonic coaching is a supportive option especially for employees who have chosen a self-directed approach to achieve their behavior modification goals. The frequency of follow up can be customized; most clients select quarterly or semi-annual reviews. These coaching sessions usually take 15 to 30 minutes and can be scheduled by employees using the Summit Health proprietary online appointment system.
Coaches review steps taken, accomplishments, and barriers to achieving goals, revising wellness goals if appropriate. Follow up coaching is easy to schedule and can be used as an incentive with rewards for employees who follow through. Attendance at follow up coaching can be tracked and reported electronically to one or more incentive programs.
During these coaching calls, Coaches speak directly with participants, not family members or others. Telephone messages are left only if participants provided permission to do so. The coaches’ goal is to ascertain what actions have been taken by employees, the outcomes and the most appropriate next steps. Their approach is to be supportive and to obtain a commitment from employees to achieve the next steps to reach their goals.
