The ability of organizations to operate their wellness programs and to achieve their goals is dependent on the quality and type of the data collected. Many organizations use biometric screening results and health risk assessment scores to make decisions that affect employees and the company’s bottom line. More and more of our clients have integrated these results into their health plan, disease management programs, and employee incentive programs. Corrupted or inaccurate data compromises the effectiveness of such programs. Summit Health has a robust solution to facilitate and expedite data collection, accuracy analysis and secure data integration with third party vendors such as health plans.

Summit Health’s data solution includes

  1. Automation for entry of results (Scan-in forms versus keying in data)
  2. Barcode on forms that connect to eligibility files
  3. Data validation when entered into the system and a correction process
  4. Image of Informed Consent form and data, paper copies shredded for security purposes
  5. Multiple years of data collection and reporting capability
  6. Store data on servers located in a SAS 70 Type II facility
  7. Year-to-Year Aggregate report (Rolling 5 years) by Company, Location, and Employee -- A standard report
  8. Robust turn around on data and aggregate reports
  9. Pre-population of Health Risk Assessment (HRA)
  10. Integration of with Health Risk Assessment (HRA) data
  11. Portal employees can log into to view their screening results along with graphs showing trends

Customized Data Collection

Informed Consent Form

Summit Health works with clients to customize an informed consent form (see below) that includes all the personal data that need to be collected along with the biometric results. This personal data can include address, employee ID, and lifestyle questions such as, “Do you smoke?” and “Do you want a health coach to contact you?”

The informed consent forms are sent to our corporate headquarters using a secure method. At the corporate headquarters the forms are scanned into a relational database using our scantron system that reads information using Optical Character Recognition (OCR) and Optical Mark Reader (OMR). Information from the informed consent form (employee name, address, employee number) and biometric information are loaded into the Summit Health secure relational database. As the data are scanned, the system checks the upper and lower bounds of the fields as a validation check. When a field fails the validation, the scanner is stopped and the staff addresses the data issue.

HIPAA Compliance & Data Security

Personal Health Information is protected during data storage and transfer to HIPAA-compliant parties. Summit Health has a dedicated Server which is hosted at a secure Facility in Novi, Michigan by Secure 24 (SAS 70 Type II Certified facility), where we store our HIPAA data. The facility is a high-security hosting center with armed security guards, state-of-the-art firewall, redundancy, and daily back-up. This hosting facility has intrusion and attack prevention and detection capabilities, and tracks unauthorized access and attempts to access the secure data. Summit Health’s Information Technology systems have been reviewed in detail for security and HIPAA compliance by our health plans and Fortune 500 clients.

Individual screening results along with all reports are stored on this secured server. All information associated with Internet-based appointments is also stored on the secured server. All consent forms are scanned and the image stored on the server so no paper informed consent forms need to be stored.

Our internal business application and email servers have RAID 5 hard drive redundancy and are backed up daily on a set of two hot-swap removable hard drives. These backup hard drives are swapped out weekly and stored offsite at our secure offsite warehouse, which is fireproof, waterproof, and guarded 24x7. In addition, key server data is backed up daily via a secure Web service for added redundancy.

Summit Health also trains our staff to keep a patient’s HIPAA information confidential during the biometric screening process. Security policies and procedures Summit Health employs:

  • All of our employees and field staff sign a confidentiality agreement
  • Data collected at health screenings do not include the participant’s name
  • Screening results are not disclosed aloud at the screening but are written on our proprietary counseling brochure and are pointed to.
  • Data entry is conducted in a special room with a security lock
  • The results sheet given to each participant does not have their name on it in the event it is lost by the employee

Summit Health provides electronic reports to organizations such as Aetna, CIGNA, Humana, Kaiser Permanente, UnitedHealthcare, Health Dialog, Matria/Alere, SHPS and 22 Blue Cross Blue Shield plans. Summit Health is flexible in file layout and data format, and we work with clients to define a format that meets their requirements.

Summit Health works with the Information Technology departments of our customers to define data transmission and integration. Our Information Technology team of twelve staff has met all of our customer’s requirements and requests.

Summit Health works closely with third party organizations in a seamless manner. Summit Health is able to do this based on our large service offering, state-of-the-art Information Technology Systems, and our single-point-of-contact program.
