As a member of Summit Health's Behavioral Health team, Heidi Schnapp, MA, LAC, CSW, CMC is dedicated to the care and support of oncology/hematology patients. Ms. Schnapp is a sought-after facilitator who offers community-building support groups throughout northern New Jersey. She has over 25 years of experience advocating on behalf of older adults, individuals with disabilities, and patients and their caregivers.
Ms. Schnapp has a strong interest in aiding those living with Parkinson’s disease, a passion that led her to pursue and implement a new hospital initiative at Saint Barnabas Medical Center for those fighting the disease. Her interest in this area continues with volunteer work at the Randolph YMCA, where she offers support groups and programming assistance on issues related to Parkinson’s disease and aging. Ms. Schnapp’s advocacy has been recognized by the American Parkinson's Disease Association NJ Chapter and the Michael J. Fox Foundation.
Additionally, Ms. Schnapp’s is a co-founder of LMR Elder Care, LLC where, as a professional geriatric care manager, she helps families navigate the elder care maze by guiding them toward informed decisions to maintain dignity and quality of life. Heidi has held positions on the American Cancer Society Board of Advisors for Northwest New Jersey, Mental Health Addictions Services Advisory Board of Morris County, and Mental Health Association of Essex County Board of Directors.
