• Horizon BCBS has partnered with Novartis and Summit Medical Group to offer an innovative quality program for patients with chronic medical conditions such as diabetes, or  musculoskeletal, cardiovascular or digestive disorders.
• Horizon and Novartis have partnered with Summit Medical Group because SMG has delivered positive patient outcomes including lower hospitalizations and greater use of preventive services that keep patients healthier
• Novartis employees who are candidates for this program must be invited to participate in the program.
• Invited employees who elect to participate and use any Summit Medical Group provider pay a reduced copay for their SMG visits.
• Patients receive an ID card from Novartis which they must present in order to receive the reduced rate
