Program Overview

The PGY2 residency in Ambulatory Care builds upon PGY1 residency graduates’ competence in the delivery of patient-centered care and support of optimal medication therapy outcomes. The program prepares its residents to become advanced, skilled pharmacy practitioners who are qualified for a position as an ambulatory care clinical pharmacy specialist. Based in New Jersey, this dynamic, hybrid program provides remote & in-person training in physician-run clinics within an integrated healthcare delivery system and accountable care organization (ACO). We offer longitudinal experiences with a focus on chronic disease pharmacotherapy, population health management, and value-based care initiatives. Graduates from this residency program will be well-suited to run their own ambulatory care service in multiple settings and specialties.

Curriculum

Most of the residency year is divided into telehealth experiences (2 days in New Providence office & 2 days remote) and in-person patient care (1 day in Warren Primary Care Clinic). In addition to direct patient care clinic activities, the resident will lead a novel telehealth service with a focus on endocrinology & cardiology; participate in a pharmacy consult service; and develop their own telehealth panel of patients. Furthermore, the resident will present at various meetings; manage reports for ACO quality measures; contribute to care management & transitions of care; develop organization-wide protocols and policies; contribute to cost-saving specialty pharmacy initiatives; and engage in research.

Application Process

Interviews usually begin 2-3 weeks after the application deadline

  • Interview may be on-site in New Providence, NJ (preferred) or virtual (upon request)
  • Interview will require a short presentation and patient case assessment
     

ASHP Match schedule program compliance

  • Start date: July 6, 2026
    • NJ pharmacist licensure must be obtained within 6 weeks of start date.

FAQs

Summit Health is part of an accountable care organization (ACO) within the Centers for Medicare & Medicaid Services (CMS) called “The Shared Savings Program” (aka MSSP). MSSP is a voluntary program that encourages groups of doctors, hospitals, & other health care providers to come together as an ACO to give coordinated, high-quality care to their Medicare beneficiaries. ACOs are held accountable for performance on outcomes, patient experience measures, and cost of care for Medicare beneficiaries. The Clinical Pharmacy team works with providers, nurse care managers, and patients to optimize these outcomes, such as medication adherence & statin use. The resident will have the unique opportunity to manage reports for ACO & HEDIS quality measures.

The program is based in New Jersey, with our office location in New Providence & clinic location in Warren. Residents are allotted 2 days per week for remote work (WFH). For the Community Health rotation, the resident is expected to travel to community outreach sites in northern NJ (e.g., Montclair, Orange, Jersey City) a minimum of 6 times per year. The resident is expected to manage his/her own transportation among these sites.

The PGY2 Ambulatory Care Pharmacy residency program conducted by Summit Health, New Providence, NJ has an accreditation candidate status with ASHP.

No. Residents work Monday through Friday, with no required weekends nor holidays. There is no staffing component.

Yes, the resident will attend the ASHP Midyear meeting and a regional resident conference. Financial support will be provided for these meetings.

Residents may provide in-services to clinic staff as well as present (typically virtually) at P&T, care management, and other department meetings. We do not offer a teaching & learning certificate (TLC) program. The resident will not precept students nor PGY1 residents.

Yes, you are required to be licensed within 6 weeks of the residency start date.
